Welcome to the Shin Kaze online registration form for members who are Yudansha but may be new or returning dojo members but not already in the SK Member Database. Before completing this form for a dan registration, please take some time to ensure that you have all the required information for the candidate in order to ensure that the form can be submitted.
INSTRUCTIONS
THE OVERALL PROCESS
The overall process is as follows.
- As dojo-cho, you fill and then submit this form to process your students’ dan registrations.
- After submitting the form, the information it contains is validated and if any issues are found, you are prompted to correct them.
- Once all the information has been validated, it is saved in the database as a record.
- The process is repeated for all dan level members to be registered and is not a substitution for a dan promotion request which is required to be sent to Hombu Dojo please use the correct form in that case. Once registered, you will be sent an invoice for the members SK membership which will be pro-rated based on when in the year the registration is done.
- Once you receive the bill please pay promptly, and your Dan rmember will be registered and can in future then be a candidate for promotion.
DATA INPUT
- Complete this form for each student to be registered, and then click on the “Submit” button.
- Not all fields are *required, however wherever possible please complete even non-required fields. Please make sure all fields that are *required are accurate, especially dates. Fields 1, 2, 3, 4, 5, 6, 8, 9, 10, 11, 12, 15, 16, 17 18, and 19 are required in order for the member to be registered and for future promotions.
- The Dojo Name and Dojo ID# are really important, as they connect the student’s information to the dojo. Make sure you select the correct Dojo ID# to match the one in theDojo Name field just above it.
