Dear Dojocho, welcome to the Shin Kaze online Dojo Roster Request Form. Just follow the instructions and once approved you will receive an email with an excel spreadsheet that includes all the members of your dojo.

THE OVERALL PROCESS
Just complete the form below.

  1. As dojo-cho, you fill and then submit this form to view the existing data that is in the database.
  2. After submitting the form, once it is verified that you are eligible to receive the dojo roster an email with a link to an online form to request data on a specific member of your dojo.
  3. Once you have viewed the data of the specific member, a process is available to update or make changes to that record.
  4. The process is repeated for all students to be viewed that you may wish to update prior to a promotion or maybe just a returning member who you wish to change their status form inactive to active.

DATA INPUT

In order to see the returned data, a confirmation email will be sent to you once you submit the form below that will include a link to see what data is in the database for that member. If updates are required please copy and paste the data, update the fields as required and send to the SK Customer Service email at: shinkazecustomerservice@gmail.com

Once the changes to the record for that member are reviewed, the database will be updated and a confirmation email of those changes will be returned to you for your records.

The Information Management Committed (IMC) is working to simplify and streamline this administrative processes for the benefit of our member dojos.  Please follow these simple instructions in order to ensure that any search and update to the members information is accurate.
